2 Case 1: Bệnh nhân: Nguyễn Khắc H, Nam, 53 tuổi (STT:190201008)
Địa chỉ: Bình Dương Ngày khám ở ĐKHH-MEDIC: 01/02/2019 Lý do đến khám: Đau ngực trái khoảng 1 năm Bệnh nhân được chỉ định làm xét nghiệm: ECG, XQ phổi thẳng, siêu âm tim, siêu âm bụng tổng quát và chụp MSCT mạch vành.

6 MSCT MẠCH VÀNH Hẹp 30% LAD I và 30-40% LAD II. Hẹp 20-30% RCA II.

7 Động dưới đòn phải xuất phát lạc chỗ từ động mạch chủ xuống, vòng từ trái qua phải, đi trước cột sống và sau khí quản, thực quản. Tồn tại lỗ bầu dục (d#3.5mm)

Địa chỉ: Tây Ninh Ngày khám ở ĐKHH-MEDIC: 15/01/2019. Lý do đến khám: Bệnh viện Phạm Ngọc Thạch chuyển đến chụp MSCT mạch vành với chẩn đoán thiếu máu cơ tim.

12 Quai động mạch chủ bên phải – Túi thừa Kommerell – Xuất phát bất thường động mạch dưới đòn trái từ động mạch chủ xuống, chạy từ phải sang trái đi trước cột sống và sau khí quản – thực quản.

13 ARTERIA LUSORIA Aberrant right subclavian arteries (ARSA), also known as arteria lusoria “dysphagia lusoria”, are the commonest of the aortic arch anomalies. Hanuld was first described this arterial anomaly in 1735. The estimated incidence is 0.5-2%. They are often asymptomatic, but around 10% of people may complain of tracheo-esophageal symptoms. 80% posterior to esophagus, 15% between esophagus and trachea and 5% anterior to the trachea.

CT or MRI now usually accomplishes the definitive diagnosis. Both modalities can demonstrate the aberrant branch arising from the distal left aortic arch and coursing as the right subclavian artery. Surgical and endovascular treatment of symptomatic aberrant right subclavian artery (arteria lusoria). Although mostly asymptomatic, unspecific thoracic pain, dysphagia, dyspnea, arterioesophageal or arteriotracheal fistula and aneurysmal formation with relevant risk of rupture. 

15 KOMMERELL DIVERTICULUM
Kommerell diverticulum occur in some anomalies of the aortic arch system.  It usually refers to the bulbous configuration of the origin of an aberrant left subclavian artery in the setting of a right-sided aortic arch. It is named after Burckhard F Kommerell, a German radiologist ( ) who first described it in 1936. Kommerell diverticulum may be asymptomatic or cause symptoms of esophageal or tracheal obstruction.

16 Typically shows bulbous enlargement of the proximal subclavian artery at its origin from the aortic arch, posterior to the esophagus by CT/MRI. The major complications of Kommerell diverticulum are rupture or dissection. Given the rarity of this condition, there is no established management guideline. Options include aortic replacement or thoracic endovascular stent-graft replacement (TEVAR), and extra-anatomic bypass of the aberrant subclavian artery.

17 KẾT LUẬN Động mạch dưới đòn lạc chỗ và túi thừa Kommerell là bất thường bẩm sinh hiếm gặp. Đa số bệnh nhân không có triệu chứng lâm sàng và tình cờ phát hiện do đau ngực, khó thở hay khó nuốt. Nhờ chụp MSCT mạch vành và mở rộng phần CT ngực sau khi bơm thuốc cản quang, giúp phát hiện được những bất thường này.
